Post Incident Review
Incidents and accidents happen – the key is to learn from them to prevent re-occurrences. Too often organisations:
miss the early warning signs that can prevent more serious events, or
misdiagnose the cause and leave unresolved the true root cause of an incident.
So what is “an accident” are they truly accidental? Our experience shows that most of the time there are controllable causal factors that are present. With this in mind, reviewing and investigating incidents to isolate the true root causes is a fundamental element in any system.
The investigation objectively assesses the incident and offers a conclusion which contains constructive recommendations to prevent the situation occurring again. Our investigations consider both system errors and human factors that contributed to the incident. There are several investigative methods used, depending on the type of incident, but all conclude with tangible interventions to reduce the probability of a repeat occurrence.
Our staff have written and reviewed operational safety systems for exploration and expedition organisations, and have been responsible for putting together industry standards on safety management. We have the technical ability to review incidents, investigate why they occurred, and give advice on reviewing operational systems accordingly.
